Conventional care for suicidal individuals follows long-established traditions; these are generally viewed as standard of care – and are seriously flawed. The conventional model makes great common sense (especially in legal circles) but is destined to fail unless and until the fact is fully acknowledged that the agenda often is not shared by the patient. This presentation will review the conventional approach and present an alternate approach that prioritizes a collaborative working relationship combined with suicide-specific strategies with strong evidence of superiority to usual care. Ethical implications of this issue will be explored briefly, together with practical pointers for improving outcomes in both the near term and the long run.
